Medical Exceptional Family Member Program - EFMP
What is the EFMP?
The EFMP is a program offered by each service to support families with special medical and educational needs throughout the service member’s career. Family members should enroll in EFMP when a qualifying condition is diagnosed. Each branch has its own EFMP and enrollment process.
EFMP family support services are available at your installation. The family support services team is your main point of contact for EFMP information.

JBER SCMH Primary Care Window -Service members must work with their currently assigned EFMP Medical/Family Travel Office, for all questions and concerns. Please do not contact projected or gaining offices. Your assigned offices can provide updates and answer any questions or concerns that you may have. You must coordinate with your assignments office, branch manager, and HRC to push report dates if you have not started or completed the EFMP process. If you have not started or completed the EFMP process, do NOT continue with your PCS, sell/buy/rent your home, ship your goods/vehicle, move your family to an OCONUS location without completing EFMP process for authorization, otherwise doing so may result in substantial federal debt incurment without eligibility of reimbursement, rescinded orders, interception at airports and borders, forfeiture of additional benefits, etc. Command sponsorship in OCONUS locations is not automatic and not guaranteed. Do not try to bypass the EFMP process with the misconception that you will be able to complete the EFMP process upon arriving at your OCONUS location. You will be assuming the aforementioned risks.

Q: What is the Exceptional Family Member Program (EFMP)?
A: The Exceptional Family Member Program (EFMP) is a mandatory enrollment program that works active- duty military Families who have a Family member that has been identified as having special medical and/or educational. The EFMP coordinates with other military and civilian agencies to provide comprehensive and coordinated community support, housing, educational, medical, and personnel services to Families with special needs. Soldiers on active duty enroll in the program when they have a Family member with a physical, emotional, developmental, or intellectual disorder requiring specialized services so their needs can be considered in the military personnel assignment process. The overall goal of EFMP is to assist in the mission readiness by providing comprehensive care and services to Army Families with special needs dependents and to help Families accompany the Service member to the right duty locations, not to exclude them.
Q: What is an Exceptional Family Member?
A: An Exceptional Family Member is a Family member (child or adult) with any physical, emotional, developmental, or intellectual disorder that requires special treatment, therapy, education, training, or counseling.
Q: Who must enroll in the EFMP?
A: IAW AR 608-75 Sections 1-7. Identification and enrollment a. Enrollment in EFMP for Soldiers with exceptional Family members (EFMs) (children and adults) is as follows: (1) Active Army Soldiers will enroll in the EFMP. (2) U.S. Army Reserve (USAR) Soldiers in the Active Guard Reserve (AGR) Program will enroll in the EFMP. (3) Army National Guard (ARNG) Soldiers in the Active Guard Reserve (AGR) Program will enroll in the EFMP. (4) ARNG and USAR Soldiers not serving in the AGR status may voluntarily enroll in the Army EFMP before entering an eligible duty status to gain access to the EFMP immediately upon entering a status that is eligible to receive EFMP services (for example, community support services such as EFM information, referral, and placement; advocacy; family-find activities; and respite care) The Soldier's eligible Family members must meet the special needs medical eligibility criteria in AR 608-75, appendix B, when the Soldier is in an eligible duty status.
Q: How does a Soldier enroll in the program?
A: The Soldier contacts the nearest Army medical treatment facility EFMP point of contact to begin the assessment process and obtain the enrollment forms. Once the forms are completed, they are forwarded to the appropriate regional medical command for coding. The regional medical command enrolls eligible Active Army Soldiers in EFMP. They notify Army National Guard and Reserve personnel agencies of eligible AGR Soldiers who then enroll them in the program.
Q: How Does a Soldier disenroll from EFMP?
A: Once a Family member is enrolled in the EFMP, he/she remains in the program until: The condition that warranted enrollment has been resolved; When the Soldier leaves the Army; or When the Family member is no longer enrolled in DEERS. Documentation is required to disenroll from the EFMP regardless of the circumstances and this documentation must be requested through the either the medical provider for a medical enrollment or the school/early intervention provider for an educational enrollment.
Q: What is the Enterprise Exceptional Family Member Program (E-EFMP)?
A: The Enterprise Exceptional Family Member Program (E-EFMP) is a new system that was launched in August 2022. The E-EFMP system makes it easier and more convenient for Service members and their Families to navigate the wide and sometimes complex array of resources, benefits, and care available. Soldiers can complete EFMP enrollment, reenrollment, and overseas Family Member Travel Screening (FMTS) if Soldier and family member are co-located and Soldiers and Families that are geographically separated using the E-EFMP system. The E-EFMP system has disenrollment capabilities, builds digitalized PDF DD form 2792, DD form 2792-1, DA form 5888 once a package has been initiated and is available to print or save once the package is complete.
Q: How do I check if I have family members enrolled in the EFMP and where do I find the enrollment expiration date?
A: Using your DS Logon or CAC to login the E-EFMP website, “My Account” to view family member status and enrollment expiration date/disenrollment date.
Q: How do I get status updates on my package?
A: Using your DS Logon or CAC to login the E-EFMP website, “My Account” to then click on “Package History”. The last timestamp is where the package is currently sitting waiting for the next person assigned in the workflow algorithm.
Q: Does enrollment have any impact on the Soldier’s career?
A: Enrollment in EFMP does not adversely affect selection for promotion, schools, or assignment. Information concerning enrollment in EFMP or any of the data used in the program is not made available to selection boards.
Q: What are the benefits of enrollment in the program?
A: Enrollment allows assignment managers at Army personnel agencies to consider the documented medical and special education needs of EFMs in the assignment process. When possible, Soldiers are assigned to an area where the medical and special education needs of their EFM can be met. This will depend on a valid personnel requirement for the Soldier’s grade, specialty and eligibility for the tour. All Soldiers are still eligible for worldwide assignments.
