Q: How long does it take to get my medical records?
A: We complete records request in 30 business days or less. 
Q: Can I request my records from another facility?
A: Yes.  You must come in to Patient Administration to fill out request for records paperwork.
Q: Can I request a copy of any radiology scans?
A: No. You must request those from the radiology department directly.
Q: Do I need an ID card to receive service?
A: Yes.  A valid ID card is required to verify eligibility for services.
Q: Is there a cost for a copy of my medical records?
A: The first copy of your medical record is free. There is a small fee for a complete second copy.
