Q: How long does it take to get my medical records?
A: Depending on the amount of requests received, it can take as little as five business days or up to 30 business days.
Q: Can I request my records from another facility?
A: Yes. You must complete a DD Form 2870 requesting requesting the other medical facility to release the records to Bassett ACH.
Q: Can I request a copy of any radiology scans (actual x-ray, CT Scan, etc.)?
A: No. You must request those from the radiology department directly. Our releases only pull and produce the interpretation and report of the scan.
Q: Do I need an ID card to receive service?
A: Yes. A valid ID card is required to verify eligibility for services.
Q: Is there a cost for a copy of my medical records?
A: The first copy of your medical record is free. There is a small fee for a complete second copy. We produce and disseminate medical records via DODSafe or compact disk. Please ensure you annotate on the DD Form 2870 on the route preferred.
