Q: How do I print a copy of my profile?
A: Go to MODS homepage at www.mods.army.mil select Medical Readiness Portal to get a copy of your profile.
Q: How do I complete the online portion of a PHA?
A: Go to MODS homepage at www.mods.army.mil, select Medical Readiness Portal. Under Self Service, select Periodic Health Assessment to complete portion 1 of PHA.
Once complete, you are ready to schedule your face to face with your provider by calling (907) 384-0600.
Q: How do I enroll in MHS GENESIS Patient Portal?
A: If you previously had a TRICARE Secure Messaging account, your account will automatically transfer over. If you did not previously have an account, please create one at the MHS GENESIS Patient Portal log-in page.
