Birth Registry


Birth Certificates
The State Department of Health requires that all births be filed within one week of the child’s birth.

Parents need to complete the ALASKA VITAL RECORDS PARENT Worksheet provided by the maternity ward at time of birth. Within 5 days it must be given to the PAD Office for submission to the State Department for registration.

*Affidavit of Paternity if required must be obtained from the PAD office, if you are unsure if you need this, please inquire with PAD*

PAD will provide a copy of the Alaska Vital Records Parent Worksheet and a Certificate of Live Birth to the active duty service member. The active duty service member must take these documents to their personnel office and enroll the newborn child into DEERS temporarily.

Once registered, parent(s) may order a certified copy of the child’s birth certificate from the Alaska State Department, for all current fees and processing times please visit www.vitalrecords.alaska.gov .

You must request certified copies of the birth certificate from the State Department of Health, Vital Records Section. The active duty service member must take this to your personnel office. Certified copies of the birth certificate are needed to keep the newborn on DEERS permanently.
