Referrals:
Self-referral
Patient can self-refer at any time by calling the behavioral health clinic front desk. Please read the stipulations for what qualifies for mandatory treatment and voluntary treatment. (Self-referrals must take place prior to being notified of a pending urinalysis or it will be changed to a command referral) Self-referrals whether enrolled in mandatory or voluntary treatment are covered under the Limited Use Policy and are not subject to adverse administrative action .
Medical Referral
Medical providers can refer patients to SUDCC through CHCS. Please contact the SUDCC SSA or NCOIC with any questions. Referrals should take place if there are any noted problematic substance abuse criteria discovered during routine or emergency medical screening.
Command Referral
Commanders must refer service members with a substantiated positive drug test or other alcohol or drug related incidents within 5 days of receipt of the notification. Command Teams refer service members to SUDCC using DA Form 8003. Forms are available at the behavioral health front desk or Command Teams can contact the SUDCC SSA or NCOIC to receive the form via email.
Mandatory enrolled substance abuse treatment is determined during clinical assessment. If it is determined that a substance use disorder diagnosis is present the service member will be enrolled in mandatory substance abuse treatment if the diagnosis is related to illegal drug use, including illicit use of prescription drugs. If alcohol use is determined by law enforcement, apprehension, and/or an alcohol breath or blood test indicates alcohol impairment while on duty and the assessment determines the presence of a substance use disorder then the enrollment will be mandatory. If after assessment it is determined that the service members has clear effects on judgment, reliability or trustworthiness, or presents a clear risk to safety of themselves, others, or the mission then the enrollment will be mandatory.
Service members will be eligible to receive voluntary alcohol related behavioral healthcare if, after clinical assessment that the service member does not meet any of the criteria for placement in mandatory substance abuse treatment.
